Please note:
Children must be accompanied by a parent/legal guardian with documentation.  Each child must have their immunization record with them.  
All shots are being provided on a first come, first served basis.  
Please arrive 30 minutes before the program's end time to ensure your spot.  
This program is geared towards the uninsured.
