Please note:
Children must be accompanied by a parent/legal guardian with documentation.  Each child must have their immunization record with them.  
All shots are being provided on a first come, first served basis.  
Please arrive 30 minutes before the program's end time to ensure your spot.  
This program is geared towards the uninsured.

Las Vacunas Pfizer para los niños serán proporcionadas por el Departamento de Salud del Condado de Suffolk. 
SCDH ofrece , según el suministro asignado.
Tenga en cuenta:
